Professional Partner Participation and Attribution Agreement

PROFESSIONAL PARTNER PARTICIPATION AND ATTRIBUTION AGREEMENT Version v1.1 Last Updated December 28, 2025 1. Purpose and scope This agreement governs participation in the Monetize MD routing system. Monetize MD facilitates access to third party programs and services and manages attribution and revenue participation. Monetize MD does not provide consumer services and does not control fulfillment outcomes. The Partner engages Monetize MD to monetize referred demand without expanding scope or assuming delivery obligations. 2. Role clarity Monetize MD role. Monetize MD operates as a commercial intermediary. It establishes referral pathways, executes attribution tracking, issues reporting, and distributes revenue participation according to agreed terms. Execution partners. All services are delivered by independent third party providers or operators who contract separately with Monetize MD. The Partner has no authority over fulfillment decisions, pricing changes, or service delivery. 3. Revenue participation The Partner is entitled to revenue participation from transactions attributable to Partner referrals as defined in Schedule A. Revenue participation is transaction based. If no transaction occurs, no compensation is owed. Attribution is lifetime for each referred participant subject to continued program availability and system operation. Monetize MD may participate separately in revenue arrangements with execution partners including platform fees or ongoing participation. 4. Program modularity Monetize MD may add, remove, or modify Program Modules over time. Each Program Module may have distinct economics, entry conditions, and compliance boundaries. Participation in any Program Module is optional. Monetize MD may add, remove, or modify Program Modules without amending the core agreement. 5. Compliance and non representation The Partner agrees not to represent themselves as providing medical care, wellness services, diagnostics, or treatment unless independently licensed and contracted to do so outside the Monetize MD system. The Partner agrees that representations to prospects must remain within referral and educational boundaries defined by Monetize MD. 6. Independence and non exclusivity This agreement does not create employment, partnership, agency, or exclusivity. Either party may terminate participation subject to notice provisions set by Monetize MD. Attribution treatment after termination is defined in Schedule A. 7. Confidentiality and system protection Economics, attribution methods, program structures, and execution relationships are confidential.
